Interested applicants should submit - Two (2) passport size pictures -Letter of Acceptance from the university or Registration form as certificate of enrollment -True copy of grades for the entire High School or Transcript of Record -Two (2) sealed reference letters from high school authorities -Medical History and Certificate from family doctor -Application form (available at the Reception or print it here) After submission of the requirements, an appointment can be set for the personal interview with the applicant (accompanied by parent/s).
